Step 1: Open the personal email account you supplied to CannaMD.
Step 2: Locate and open the emails with “Medical Marijuana Use Registry New User Account” as the subject line. One email contains your username (your username is the email you supplied to your doctor). The second email contains your temporary password.
Remember your temporary password, you will use it to log in!
Passwords in the Registry are case sensitive. Temporary passwords are in the following order: 3 capital letters, 2 lower case letters, 3 capital letters, a symbol and a number. For example, ABCdeHIJ#0 (please note that the lower-case L and a capital i look the same).
Step 3: Click on the Registry link provided in the email. This link takes you to the Registry’s home screen (website).
Step 4: Click “Log In.”
Step 5: Type your username and temporary password in the correct boxes on the screen. The temporary password is case sensitive, it is only valid for one successful login attempt and must be used within 24 hours of it being sent to you.
Step 6: Click “Log In.”
Step 7: After you log in, you will be instructed to change your password. Read the message that will appear on your screen: “Your password has expired, it must be changed.”
Step 8: Type your temporary password into the “Current Password” box (this is the temporary password that was sent to your email in Step 3).
Step 9: Create your new password. You need to add a password to your Registry account that is unique to you. It must be 10 characters long and include each of the following:
- Uppercase letters (A, B, C …)
- Lowercase letters (a, b, c …)
- Numbers (1, 2, 3 …)
- Special characters (for example: ‘#&*^$() &? /)
Step 10: Click “Change Password.”
Step 11: A message box will pop up on your screen stating, “Your Password Has Been Successfully Changed.” Click “Ok.”
Step 12: You will automatically be redirected to your Registry home page.

Step 1: Log in to the Medical Marijuana Use Registry.
Step 2: Navigate to the “YOUR CARD” menu option at the top of the page to view your identification card application.
Your photograph is imported directly from your Florida Driver’s License or Florida Identification card. If the photograph from your Florida Driver’s License or Florida Identification card did not automatically import, please contact CannaMD and confirm we entered your social security number and date of birth correctly.
Step 3: Upload a copy of your Florida driver’s license or Florida identification card under the Proof of Residence section. Click “Choose File.” Select the file you wish to upload and click “Open.” The document will appear below the “Choose File” button.
Step 4: Once you have submitted a current copy of your proof of residence, you must electronically sign your application. Scroll to the bottom of the page and type in your first and last name in the appropriate boxes.
Step 5: Click on “Submit My Card Application.”
Step 6: Click the button that reads: “Click Here to Pay Online,” which will appear after you submit your card application.
Step 7: A new window will open containing the Bill2Pay interface, where you can supply your payment details. Payments by e-check and credit card are accepted. A $2.75 convenience fee applies to each online payment.
You can also send your payment by check or money order, made payable to Florida Department of Health. You may mail in your payment to the following address:

Print the confirmation of the payment for your records after finalizing your payment. You can check the status of your application as it is reviewed.
Once your identification card application has been approved, you will then be able to contact one of the licensed
medical marijuana treatment centers and fill an order.

By Florida law, Registry Identification Card applications require specific information and documentation to prove Florida residence. Review the requirements below to be sure you have the correct documentation before you begin the application process.
Adult Applicants (Patients and Caregivers)
A valid Florida driver’s license or identification card
- If you are applying online, make sure to have a JPEG or PDF copy of your valid Florida driver’s license or ID. You will upload this to your application.
Seasonal Residents
If a seasonal applicant does not possess a valid Florida identification card or driver’s license, they must submit a JPEG or PDF copy of two of the following documents:
- A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement.
- One proof of residential address from the seasonal resident’s parent, step-parent, legal guardian or other person with whom the seasonal resident resides and a statement from the person with whom the seasonal resident resides stating that the seasonal resident does reside with him or her.
- A utility hookup or work order dated within 60 days before registration in the medical use registry.
- A utility bill, not more than 2 months old.
- Mail from a financial institution, including checking, savings, or investment account statements, not more than 2 months old.
- Mail from a federal, state, county, or municipal government agency, not more than 2 months old.
Minor Patients
- A certified copy of a birth certificate or a current record of registration from a Florida K-12 school, and
- A copy of the minor patient’s parent or legal guardian’s valid Florida driver’s license or identification card.
For all forms of residency, the name and address on the document(s) provided must match the name and address provided in the application.
